Order Your Official Transcript or Replacement Diploma

LSU Health Sciences Center New Orleans has retained Parchment® as its trusted agent for processing all online credentials requests.

 

Log in or create an account with Parchment®.

If it is your first time using Parchment®, you will be required to sign the consent form. The Family Education Rights and Privacy Act requires our institution to have a signed and dated consent form before issuing credentials. To place your order, follow the instructions, enter a recipient, and select a delivery method. When your order is submitted, a fulfillment request is sent the Office of the Registrar where it will be processed. If you enter data that does not match our records, your request will not be processed.

A transcript is $7.25 per recipient. Diploma prices vary depending on your needs. 

  • Digital Diploma - $25
  • Digital & Paper Diploma - $40
  • Notarized Diploma - $50
  • Apostilled Diploma - $70
  • Apostilled & Notarized Diploma - $80

For express delivery of printed credentials, LSU Health Sciences New Orleans uses FedEx. The additional fees associated with next day express delivery are as follows: 

  •   Within the Continental United States - $25.00
  •   Alaska and Hawaii - $40.00
  •   International deliveries - $45.00

Visa, MasterCard, American Express and Discover are all accepted methods of payment. Parchment® uses the most current security available to protect your credit card and personal information.

Different credentials take different times to process.

Electronic credentials are typically delivered when they are processed. Printed transcript requests are typically mailed within two (2) business days. Printed certificates/diplomas may take up to 21 days to process. Please allow 3-5 days for USPS domestic delivery. Express delivery for printed credentials is available. 

Unofficial transcripts can be accessed in the Self-service portal.

Both current students and alumni can access their unofficial transcript in the self-service portal. Be aware that alumni can access self-service for a period of five (5) years beyond their separation date. Log in to self-service using your network login ID. If you do not remember your network login ID, contact the LSU Health Sciences New Orleans Help Desk at (504) 568-HELP (4357) or (800) 303-3290. Only official transcripts are issued after the five-year period.

Certified Diploma Copy for Licensure

Certified diplomas for licensure are provided to licensing boards free of charge if you provide a photocopy of the diploma, or a copy is on file at the university. Complete this form, then return it to our office by email, fax or in person. If there is not a copy on file, you must request a replacement diploma online through Parchment®. The cost is $30 per copy. Please allow up to ten (10) business days for processing.
